Guidelines:

The poorest hearing ear has an unaided Pure Tone Average (PTA) threshold of 65dBHL or worse.

There are no criteria for the contralateral ear. The better-hearing ear can have normal hearing.

Appropriately fit and up-to-date hearing aids are currently worn.

In the case of single-sided deafness, a CROS or BiCROS, and/or a soft band or sound arc bone conduction device has been trailed.

Phoneme scores of 55% or poorer.

In the case of sudden sensorineural hearing loss, the hearing must be stable for over six months, and the cause of the hearing loss has been thoroughly investigated.
Note: This private service and is covered under most Private Hospital Cover Health Insurance policies. Additionally, patients under DVA are also covered.
